Finding an NHS Dentist: A Challenge Depending on Your Location

Accessing NHS dental care has become a daunting task for many, and new data indicates that the situation is worsening. According to recent analysis by the Nuffield Trust, nearly 600 dental practices in England have pulled out of NHS work over the past decade. This trend is particularly alarming in regions like the South West and East of England, where the decline raises serious concerns about inequalities in healthcare access. It’s estimated that more than one in ten dental practices that used to provide NHS services have stopped, creating a near-impossible situation for some communities trying to secure NHS appointments.

To put it into perspective, NHS dental activity is down by 8% compared to pre-Covid levels. A staggering six in ten adults have not seen an NHS dentist in the last two years. And for those who tried to book a new appointment during this period, only 38% were able to do so, according to the NHS GP Patient Survey. In places like Cornwall, Devon, and Northamptonshire, the drop in NHS dental practices is particularly pronounced. In Cornwall alone, more than a quarter of practices that previously offered NHS treatment have ceased to do so, making it one of the most striking examples of the retreat from NHS dentistry.

But why is this happening? Many dentists are struggling to make ends meet on NHS contracts, leading to emotional decisions about the future of their practices. One dentist shared, “It was a really hard emotional decision, but the NHS work was running at such a loss. I had to think of my team and make it work.” This sentiment is echoed across the sector, with many practitioners feeling a breakdown of trust in the system. Even if there were new government offers, the fear of entering a new NHS contract remains high.

Access to dental care is increasingly becoming a reflection of one’s financial situation and geographical location. Some professionals have opted to go private, retire, or even transition to teaching rather than continue with NHS work. The situation is so dire that the General Dental Practice Committee warns that unless the government “fixes and funds NHS dentistry,” the crisis will only escalate.

Interestingly, while most regions in England have seen a decline in NHS dental practices, North West London, the Black Country, and North East London recorded the smallest reductions. In fact, North East London is the only area to see any increase, with one additional practice offering NHS treatment compared to a decade ago. However, this is a rare exception in a landscape where the number of NHS dental practices is falling across the board.

Looking at just the number of practices doesn’t tell the full story, as not all practices provide the same level of service. Nonetheless, even other measures, like the amount of treatment being performed, indicate that numbers are still down, albeit not as dramatically as the number of practices. The government claims it is addressing the issue by changing the dental contract and training more dentists in underserved areas. They also aim to require newly qualified dentists to practice within the NHS for at least three years.

Campaign group 38 Degrees has labeled the current dental crisis as “shameful,” highlighting the reality that people are being forced to pay out of pocket for care they shouldn’t have to.
