Dr. Nihad Fathi’s life was dramatically transformed by a treatment approach that involved not only him but also his wife, Farah. This method, known as the Open Dialogue approach, emphasizes the importance of involving patients’ priorities, maintaining continuity with the same clinicians, and engaging their loved ones. Conducted across five NHS trust areas, the trial showcased promising results; however, it did not completely prevent patients from experiencing relapses. Some critics argue that additional research is necessary to validate its effectiveness fully.

In 2020, as the Covid pandemic took hold, Dr. Fathi felt his life spiraling out of control. By the summer of the following year, work-related issues plunged him into a state of anxiety, depression, and feelings of worthlessness. “It was so much pressure, stress,” he reflected, admitting that he even contemplated ending his own life. “I caught myself like a lost child, and I didn’t know what was going on in my mind. At that stage, I didn’t know what to do…”

The Open Dialogue method allows patients to have ongoing conversations with the same clinicians, focusing on their specific needs and feelings. This approach encourages open dialogue about what support they want and how it can be achieved. “It enables you to reach a much firmer and deeper understanding of the underlying reasons for the problems,” Dr. Fathi explained. He emphasized that solutions often come from the patients themselves and their families. Moreover, he noted a significant reduction in treatment costs.

However, not everyone is convinced. Dr. Sameer Jauhar, a consultant psychiatrist, believes that the positive outcomes seen in the trial could be attributed to the extra resources allocated for it, suggesting that more extensive studies are needed to confirm the approach’s benefits. Despite the skepticism, for Farah, the Open Dialogue method was the right intervention for Nihad. “They are helping him, and then he sees a better picture,” she said, filled with hope for her husband’s recovery.

If you or someone you know is struggling with distress or despair, resources and support are available in the UK through the BBC Action Line.
